REX Medical Care welcomes you. We offers comprehensive care for patients of all ages. Our physicians and staff are able to provide a variety of health services allowing patients to receive all of their health care needs within our office.​​ We welcome new patients and can often accommodate same day appointments.​

Your First Visit

On your first visit, we will begin a permanent electronic medical record. We will ask you to complete a medical history and registration form. Please bring your insurance card and try to arrive at least fifteen minutes early for your first visit. Please bring any medications you are taking, so that we may accurately document the dosage and frequency.  If you have been under the care of another doctor, we may ask your permission to transfer some of your records, if needed.

Consultation

REX Medical Care will have the perfect physician/provider for you and your family. After your consultation, your provider will order the necessary labs, imaging, procedure and referrals as needed. 

Labs and Imaging

Labs ordered will be either drawn at our office and sent to contracted laboratory or you will be provided a lab slip to schedule with the contracted laboratory. Once results return to us, you will receive a text, email, voice-message, or a direct phone call for further instructions. 

Based on your insurance type, if imaging is required, you will either receive order slip with instructions or a referral for imaging study will be submitted to your insurance company for approval. Once approved, the insurance company will provide with location and approval number of the testing site where you will need to schedule an appointment. Once the results are received, you will receive a text, email, voice-message, or a direct phone call for further instructions. 

Procedures and Referrals

Based on the procedure type, it will either be done in office or you will be referred to a specialist for further evaluation and management. 

Based on your insurance type, you will either receive a referral slip with instructions or a referral request to see a specialist will be submitted to your insurance company for approval. Once approved, the insurance company will provide with location and approval number for the specialist where you will need to schedule an appointment.

Referral consultation notes will be reviewed for efficient coordination of care. 

Follow Up and Patient Portal

Before you leave, a follow up appointment will be tentatively set up to assure all your health care concerns addressed and allow for further continuation of care in a timely manner. 

You will also be able to coordinate your care and follow along with all the updates through you patient portal that is accessible to you via internet or phone app (Healow).
